Creation and implementation of a multidisciplinary digital care consultation for seniors in palliative care

The “P5 Palliative Care” digital multidisciplinary consultation is intended for people over 65 years of age whose illness, being serious and/or incurable, can cause situations of intense suffering for the patient and for family members and friends. Given the multifactorial nature of the patient at the end of life, it is proposed that the P5 Palliative Care consultation include the following areas: Psychology, Mourning, Nursing, Palliative Medicine, Pain, Nutrition and Caregiver Support. The general objective of the consultation is to promote the well-being and quality of life of these patients and their caregivers, regardless of the stage of the disease. For this purpose, it is proposed that the P5 Palliative Care consultation be made available in Packs, namely the Assessment Pack, Follow-up Pack and Caregiver Pack, which respectively focus on the initial assessment of the needs of the patient, after which a monthly and flexible plan within the clinical/psychological areas of care is established, with due parallel aid to the caregiver.
